Forms are necessary if you wish to gather user information. Information can be obtained through e-mail, but only if you are not looking for specific data.
On educational pages form are used most often for student log-in, creation of self-tests, and various calculations.

Designing a form does not take a lot of effort, but basic aesthetic criteria must be met.
Form design has to be clear and simple. It can be visually separated from the background by using a frame or a different colour. The most suitable are light colours, beige and white, while textures should be avoided.
A form should be short. It should not ask for unnecessary or too many information, but neither should it offer too many answer options.
A good thing to do is to include a thank you page, in order for the user to get feedback on the received questionnaire.
